On November 12, 1996 the world deadliest collision took place in Charkhi Dadri in India involving Saudi Flight 763 and Kazakhstan Airlines Flight 1907. On that fateful day the Saudi flight 763 took off as scheduled to Dhahran Saudi Arabia with 289 passengers and 23 crew members. Kazakhstan flight 1907 was on its way to Indira Gandhi carrying 27 passengers and 10 crew members.
As a result of the Kazakh pilot flying lower than the assigned clearance altitude there was collusion. Lack of adherence to simple instructions cost the lives of all passengers and crew members on board in both planes.
The Civil Aviation Authorities in India made it mandatory for all aircraft flying in and out of India to be equipped with a Traffic Collision Avoidance System (TCAS), setting a worldwide precedent for mandatory use of TCAS.
